Rehab/Insurance Membership

The Approval Letter from your insurer must be received by Knox Leisureworks before the membership will commence. Once we have received the approval from the insurer Knox Leisureworks will start the membership 
All communications regarding funds being paid must occur between the applicant and their insurer. Knox Leisureworks will not enter into any negotiations with an insurer on behalf of the applicant.

•  Approval Letter must contain:
  • The applicants name
  • Address
  • Applicant’s contact details
  • Claim number
  • Type of membership required
  • Value of membership
  • Confirmation of GP Referral
